Rare Candy 2 Increases a Pokémon’s level by 1. The amount of levels increased can be increased if the player has Candy Jars.
Potion Potion 0 – 9 Restores 20 HP or 10% HP for one Pokémon, whichever is higher. This base weight increases by 3 per party member with 87.5% or less HP, or 10 or more damage. Maximum weight of 9.
Super Potion 0 – 3 Restores 50 HP or 25% HP for one Pokémon, whichever is higher. This base weight increases by 1 per party member with 75% or less HP, or 25 or more damage. Maximum weight of 3.
Ether 0 – 9 Restores 10 PP for one Pokémon move. This base weight increases by 3 per non fainted party member with moves that have 5 or less PP remaining. Maximum weight of 9.
Max Ether 0 – 3 Restores all PP for one Pokémon move. This base weight increases by 1 per non-fainted party member with moves that have 5 or less PP remaining. Maximum weight of 3.
Lure 2 Doubles the chance of an encounter being a double battle for 5 turns.
X Attack 4 Increases the Attack of all party members by 1 stage for 5 battles.
X Defense 4 Increases the Defense of all party members by 1 stage for 5 battles.
X Sp. Atk 4 Increases the Sp. Atk of all party members by 1 stage for 5 battles.
X Sp. Def 4 Increases the Sp. Def of all party members by 1 stage for 5 battles.
X Speed 4 Increases the Speed of all party members by 1 stage for 5 battles.

Apicot Berry 2 3 Raises Sp. Def if HP is below 25%.
Enigma Berry 2 2 Restores 25% HP if hit by a super effective move.
Ganlon Berry 2 3 Raises Defense if HP is below 25%.
Lansat Berry 2 3 Raises critical-hit ratio if HP is below 25%.
Liechi Berry 2 3 Raises Attack if HP is below 25%.
Lum Berry 2 2 Cures any non-volatile status condition and confusion.
Leppa Berry 2 2 Restores up to 10 PP to a move when that move reaches 0 PP.
Petaya Berry 2 3 Raises Sp. Atk if HP is below 25%.

Full Heal 0 – 18 Heals any status ailment for one Pokémon. This base weight increases by 6 per non fainted party member with status effect. Maximum weight of 18.
Revive 0 – 27 Revives one Pokémon and restores 50% HP. This base weight increases by 9 per fainted party member. Maximum weight of 27.
Max Revive 0 – 9 Revives one Pokémon and restores 100% HP. This base weight increases by 3 per fainted party member. Maximum weight of 9.
Sacred Ash 0 – 1 Revives all fainted Pokémon, fully restoring HP. This item only restores the HP of fainted Pokémon. Only if half or more of the party is fainted.
Hyper Potion 0 – 9 Restores 200 HP or 50% HP, whichever is higher. This base weight increases by 3 per party member with 62.5% or less HP, or 100 or more damage. Maximum weight of 9.
Max Potion 0 – 3 Restores 100% HP. This base weight increases by 1 per party member with 50% or less HP, or 150 or more damage. Maximum weight of 3.
Full Restore 0 – 9 Fully restores HP for one Pokémon and heals any status ailment. This base weight increases by 1.5 per: Party member with 50% or less HP, Non-fainted party member with 150 HP lost, Non-fainted party member with status effect. A party member can count for both of the last two criteria. Maximum weight of 9.
Elixir 0 – 9 Restores 10 PP for all of one Pokémon’s moves. This base weight increases by 3 per non fainted party member with moves that have 5 or less PP remaining. Maximum weight of 9.
Max Elixir 0 – 3 Restores all PP for all of one Pokémon’s moves. This base weight increases by 1 per non fainted party member with moves that have 5 or less PP remaining. Maximum weight of 3.
Dire Hit 4 Increases the critical-hit ratio of all party members by 1 stage for 5 battles.
PP Up 2 Permanently increases PP for one Pokémon move by 1 for every 5 maximum PP (maximum 3). Can only be applied 3 times to a single move.
Super Lure 4 Doubles the chance of an encounter being a double battle for 10 battles.
Nugget 5 Grants a small amount of money. Can’t appear on wave 199 of Classic and Challenge mode.
Evolution Items 1 – 8 Evolution Items can appear if a Pokémon that can evolve by one is in a player’s party. The weight for these items begins at 1 and increases as waves are completed, up to a maximum of 8. Evolution Item Weight Formula: Weight = ⌈Wave No. 15⌉
Map 1 1 Allows the player to choose their next biome from any possible option(s). Only in classic mode.

Memory Mushroom 1 – 4 Recall one Pokémon’s forgotten move. Doesn’t apply to forgotten egg moves or TMs. Memory Mushroom Weight Formula: Weight = Highest Lv. Party Member 20 Minimum weight of 1 Maximum weight of 4.
Base Stat Booster (Vitamins) 3 Increases the holder’s stat by 10%. A Pokémon can only use as many Vitamins as they have IVs in each individual stat. For example, if a Pokémon has 31 speed IVs they can use up to 31 Carbos. If a Pokémon has 15 attack IVs they can only use up to 15 Proteins.
Tera Shard 1 Changes one party member’s typing to the tera type for 10 rounds. Has a 1/3 chance to be a random type of one of your party members, a 2/3 chance of being a purely random type, and a 1/64 chance of being Stellar.
DNA Splicers 4 Fuses 2 party members together. See (Here) for more info Only in Spliced Endless, can still only appear if you have 2 or more unspliced Pokémon in your party.
Voucher 1 Lets you spin the gacha once. Weight is 0 after rerolling.

Pokerogue Ultra Items

Big Nugget 12 Grants a moderate amount of money. Can’t appear on wave 199 of Classic and Challenge mode.
PP Max 3 Acts like 3 PP Ups. Can only be applied once to a single move.
Mint 4 Changes a single Pokémon’s nature to the nature in the mint’s title. Using a mint on a Pokémon unlocks the corresponding nature for the starter.
Rare Evolution Item 1 – 32 Evolution Items can appear if a Pokémon that can evolve by one is in a player’s party. The weight for these items begins at 1 and increases as waves are completed, up to a maximum of 32. Rare Evolution Item Weight Formula: Weight = 4 * ⌈Wave No. 15⌉
Amulet Coin 3 5 Increases money rewards from all sources by 20%.
Species Stat Booster 0 or 12 1 Only appears if you have an applicable Pokémon in your party, or in the case of the Light Ball, if one of your party members knows Fling. Boosts the following stats: Light Ball doubles Attack and Special Attack for Pikachu. Thick Club doubles Attack for Cubone, Marowak, and Alolan Marowak. Metal Powder doubles Defense for Ditto. Quick Powder doubles Speed for Ditto. Leek increases critical-hit ratio by 2 stages for Farfetch’d, Galarian Farfetch’d, and Sirfetch’d.
Toxic Orb 0 or 10 1 Badly poisons its holder at the end of the turn if they do not have a status condition already. Has a weight of 10 if a party member has the following moves or abilities: Moves: Facade, Trick, Switcheroo, Psycho Shift, Fling. Abilities: Magic Guard, Guts, Quick Feet, Marvel Scale, Toxic Boost, Poison Heal. This item will not appear if all party members that meet these requirements are already holding a Flame Orb or Toxic Orb.
Flame Orb 0 or 10 1 Burns its holder at the end of the turn if they do not have a status condition already. Has a weight of 10 if a party member has the following moves or abilities: Moves: Facade, Trick, Switcheroo, Psycho Shift, Fling. Abilities: Magic Guard, Guts, Quick Feet, Marvel Scale, Flare Boost. This item will not appear if all party members that meet these requirements are already holding a Flame Orb or Toxic Orb.
Reviver Seed 4 1 Revives the holder for 1/2 HP upon fainting. Removes all stat changes as would normally happen when a Pokémon is KO’d. Current circumstances where the Pokémon is not revived include: getting hit by an OHKO move, Weather Damage, and using Self Destruct or another similar move. Currently due to a bug, taking poison or burn damage will revive the Pokémon.
Candy Jar 5 99 Increases the number of levels added by Rare Candy items by 1. Doesn’t increase friendship gain.
Attack Type Booster 9 99 Additively increases the damage of a specific move type by 20%. Does not affect the damage of fixed damage moves like Night Shade or Metal Burst.

Rarer Candy 4 Increases all party members’ level by 1.
Golden Punch 2 5 Grants 50% of damage inflicted as money.
IV Scanner 4 3 Allows scanning the IVs of wild Pokémon. 2 IVs are revealed per stack. The best IVs are shown first. Messages correspond to specific IVs. If a stat is displayed as green in the preview it means that stat is better than what your starter has.
EXP. Charm 8 99 Increases gain of EXP. Points by 25%. Awarded after certain battles.
EXP. All 10 5 Non-participants receive 20% of a single participant’s EXP. Points.
EXP. Balance 3 4 Balances 20% of your total earned EXP. towards the lowest leveled party member(s).
Tera Orb 1 – 4 1 Tera Shards become available. For waves 1-50, the weight is 1. For waves 51-100, the weight is 2. For waves 101+, the weight is 4.
Quick Claw 3 3 Adds a 10% chance to move first regardless of speed (after priority).
Wide Lens 4 3 Increases move accuracy by 5 (maximum 100).
White Herb 3-11 2 It will restore any lowered stat in battle. Has a weight starting from 3 and adding 1 point of weight for each party member that has any moves which will lower the user’s stats or has any of the following abilities: Abilities: Weak Armor, Contrary, Guts, Moody, Anger Shell, Competitive, Defiant.

Shiny Charm 14 4 Dramatically increases the chance of a wild Pokémon being Shiny. Wild Shiny odds are 1/2048 (0.048%) by default. First Shiny Charm increases the odds by a factor of 8 to 1/256 (0.39%), additional Shiny Charms increase the odds by a factor of 2 (1/128, 1/64, 1/32).
Healing Charm 18 5 Increases the effectiveness of HP restoring moves and items by 10%. Excludes Revives.
Multi Lens 18 3 Attacks hit one additional time at the cost of a 60/75/82.5% power reduction per stack respectively. This effect works per hit with attacking moves that hit more than once.
Voucher Premium 5 Starts with a weight of 5, then subtracts a weight of 2 per re-roll, resulting in it no longer appearing after 2 re-rolls. Not available in endless.
DNA Splicers 0 or 24 Only appears if a player has two unfused party members. In Spliced Endless Mode, this item is dropped to the Great Tier Pool. * This makes the item appear much more frequently in Spliced Endless Mode.
Mini Black Hole 1 1 Every turn, the holder acquires one held item from the foe. Must be unlocked before it can appear. See this page for method.
